About the Position Position/Specialty: Pediatric Diabetic Care and Educator Specialist Perform a minimum of _5_ patient visits per day. Clinical educator position working alongside a Pediatric Endocrinologist, providing care, management, and education to pediatric diabetic patients. Position Summary: The Diabetes Educator plays a key role in the management and education of pediatric patients with diabetes and their families. This position provides individualized education and support, empowering patients and caregivers to effectively manage diabetes at home and in the community. The educator maintains their own daily patient schedule and works collaboratively with endocrinology providers, nursing staff, and the broader care team to ensure patients receive high-quality, coordinated care both during clinic visits and between appointments. Key Responsibilities: Maintain an independent daily schedule of patients for diabetes management and education visits. Provide comprehensive diabetes education to pediatric patients and their families, including disease understanding, insulin management, glucose monitoring, nutrition, physical activity, and use of diabetes technologies (e.g., insulin pumps, continuous glucose monitors). Conduct scheduled patient visits to assess self-management skills, identify barriers, and reinforce diabetes care plans developed in collaboration with the endocrinology provider. Collaborate closely with endocrinologists, nurse practitioners, dietitians, nurses, and clinic staff to optimize patient care and ensure continuity across all settings. Assist families in developing and adjusting individualized care strategies to promote safe and effective diabetes management at home, school, and during activities. Support patients and families in navigating insurance requirements, durable medical equipment, and prescription needs related to diabetes care. Provide ongoing communication and follow-up with patients and caregivers outside of clinic visits to address questions, challenges, or urgent issues as appropriate. Maintain accurate, timely documentation in the electronic medical record (EMR) to ensure continuity of care and compliance with ADA, CMS, and institutional guidelines. Participate in quality improvement initiatives, program development, and continuing education activities within the Pediatric Endocrinology department. Serve as a resource for staff education and training on pediatric diabetes care standards and best practices.
