Job Overview:
Under the direct supervision of a registered pharmacist, the Pharmacy Technician, Medication History will participate in the medication reconciliation process.
Qualifications:
Required:
High school diploma or equivalent required. Bachelor's degree preferred.
Minimum one (1) year of Pharmacy experience required, preferably within a hospital setting.
PTCB (Pharmacy Technician Certification Board) certification required within one (1) year from date of hire.
Experience:
Prior experience as a pharmacy technician is preferred.
Essential Functions:
Under the direct supervision of a registered pharmacist, the Pharmacy Technician, Medication History will participate in the medication reconciliation process.
This will include accessing the EHR to view patients in the ED that will be admitted.
The technician will interview the patient and/or family member to obtain an accurate medication history.
Review of an external medication history, contacting the patients’ retail pharmacy, or calling other physicians may be necessary to resolve any medication concerns.
In addition, they will need to update the patient’s home medication list in the EHR.
Communication skills are essential for interviewing patients and their families, as well as working with the pharmacist when patients are currently on medications that are non-formulary.
In this instance, any effort will be made to convert the patient to a formulary equivalent.
Knowledge of healthcare informatics is vital for their entry and documentation of their daily activities in the pharmacy documentation database.
